7.2Sore eyes / eye strain?

7.3Temporary blurred vision?


7.5Has the employee received any DSE health and Safety training in the past?

7.6Does the employee currently wear spectacles?

7.7Are these spectacles solely for use with DSE?

7.8When was the employee's sight last tested?
